Fraudulent Lab Testing Kits Using Our Name
We have been made aware of fraudulent activity involving unsolicited lab testing kits falsely attributed to Dr. Cooper and Elevation Medical Weight Loss. These kits were NOT ordered or authorized by our practice in any way.
If you have received an unsolicited lab kit claiming to be from Dr. Cooper or Elevation Medical Weight Loss:
Do NOT use the kit
Do NOT provide any personal information
Do NOT provide any insurance information
Do not call any phone numbers included with the kit
These kits are part of a Medicare/insurance fraud scheme. Perpetrators collect personal and insurance data to bill for tests never ordered by a legitimate physician.
